After receiving her undergraduate degree in Biology from Colorado State University, Dr. Jackson continued to Kirksville College of Osteopathic Medicine for her medical degree. She then completed a residency at the Family Medicine Center in Fort Collins, followed by a fellowship in Geriatrics at Oregon Health Sciences University.

Dr. Mast attended Colorado State University for his undergraduate studies before receiving his medical degree from the University of Colorado-Denver. He completed his internal medicine training at St. Joseph Hospital in Denver. He joined Front Range Geriatric Medicine in 2018 after 4 years in primary care in Fort Collins. “My approach to medicine is to provide compassionate, individualized care with a focus on treating each patient as if they were one of my family members.”

Dr. Glazner is a Colorado native from Cortez. She received her BS from CSU in 1983, and then served with the Peace Corps in North Africa. She earned her MD degree and MS degree in Public Health from the University of Colorado. She is an Associate Clinical Professor with CU and taught family medicine residents for 25 years, practicing at Poudre Valley Hospital for most of her career. She practices medicine in Africa when she isn’t working locally with Columbine Health.

Dr. Leible has many years of experience caring for elders beginning with her career in nursing and followed by medical school in Colorado at the University of Colorado Health Sciences Center. She did her internal medicine training and her geriatric fellowship in Colorado. During her 2 year fellowship she specialized in the care of elders in long term care and hospice. She spent several years in various administrative and teaching positions throughout the United States returning to Colorado to engage in her first passion which is caring for elders in the long term and rehabilitation setting. In her spare time Dr Leible enjoys reading, cooking and family.

Graduated from University of Wyoming with the following: 1997 Master's of Science with focus as Community Health Clinical Specialist; 2000 Post Master's Certification-Family Nurse Practitioner; 2010 Post Master's Certification-Psychiatric Mental Health Nurse Practitioner. 2012 Graduated from University of Northern Colorado -Doctor of Nursing Practice. Board Certified as both Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner.

I have a family centered philosophy of care focused on optimal, physical, and mental health functioning in those I work with. I have practiced in the inpatient, outpatient and nursing home settings both in family practice and mental health.
